Plantar Fasciitis: Nyeri Tumit di Langkah Pertama Pagi HariPlantar Fasciitis: Heel Pain With the First Steps in the Morning

KondisiConditions

Plantar fasciitis is pain under the heel caused by irritation of the plantar fascia, the thick tissue that runs from the heel to the toes and supports the arch of the foot.

Typical Symptoms

  • Sharp pain under the heel with the first steps after waking up or after sitting for a while.

  • Pain eases after walking a little, but can return after standing or walking for a long time.

  • Tenderness under the inner side of the heel.

Risk Factors

  • Limited upward ankle movement (dorsiflexion), for example from tight calves.

  • Higher body weight, especially in people who do not exercise regularly.

  • Running, and jobs with a lot of standing, especially in footwear with poor shock absorption.

See a doctor if

  • Heel pain started after a fall or hard landing.

  • The heel is swollen, red and hot, or you have a fever.

  • You have numbness, tingling or burning in the sole.

  • Pain is also present at night at rest and does not improve.

Recommended Treatment

  • Stretching of the plantar fascia and calf muscles, for short- and long-term pain relief.

  • Manual therapy to joints and soft tissue to improve movement and reduce pain.

  • Taping (rigid or elastic) with other treatment, to reduce pain in the first 1–6 weeks.

  • A night splint for 1–3 months for people who always have first-step morning pain.

  • Insoles should not be used as the only treatment, but can help when combined with other treatment.

  • Strengthening of the foot and ankle muscles.

  • Dry needling of trigger points in the calf and foot muscles can help reduce pain.

  • ESWT (shockwave therapy): a recent systematic review concluded ESWT is effective and well tolerated for this condition.

What You Can Do Now

  • Stretch your calves and soles before getting out of bed in the morning.

  • Wear comfortable, cushioned footwear. Avoid walking barefoot on hard floors.

  • Temporarily reduce running or long periods of standing, then build back up gradually.
