Cedera ACL: Operasi atau Tidak, dan Tahapan RehabilitasinyaACL Injury: Surgery or Not, and the Stages of Rehab

KondisiConditions

Apakah Harus Selalu Operasi?

Kapan Aman Kembali Berolahraga?

Peran Fisioterapi di Good Physio ID

The ACL (anterior cruciate ligament) is a ligament inside the knee that stops the shin bone sliding forward and helps keep the knee stable when you twist. ACL injuries are common in sports with sudden stops, cutting and landing, such as football, futsal, basketball and padel.

Signs of an ACL Injury

  • A "pop" felt or heard at the time of injury, usually when cutting, landing or stopping suddenly.

  • Knee swelling within a few hours.

  • The knee feels unstable or "gives way" when turning or going downstairs.

  • Difficulty fully straightening or bending the knee.

See a doctor if

  • The knee is very swollen, you cannot put any weight on it, or it looks deformed.

  • The knee locks and cannot be straightened.

  • You have numbness, a cold or pale foot below the knee. This needs emergency care.

The doctor will confirm the diagnosis and check for other injuries, such as to the meniscus or bone.

Do I Always Need Surgery?

  • Not always. In a randomised trial, active young adults with an acute ACL tear had either rehab plus early reconstruction, or rehab first with the option of later surgery if needed. After 2 years, outcomes did not differ significantly.

  • In the rehab-first group, about 61% did not need surgery within 2 years.

  • The decision about surgery is made with an orthopaedic surgeon, considering age, sport, knee instability and other injuries.

  • With or without surgery, rehabilitation remains the key.

Stages of Rehabilitation

  1. Before surgery (prehab): reduce swelling, restore full knee extension and strengthen the thigh.

  2. Early: control pain and swelling, regain knee bending and straightening, and activate the quadriceps.

  3. Strengthening: progressive loading for the quadriceps, hamstrings, hips and calves, plus balance training.

  4. Running and jumping: start once strength and movement control are adequate.

  5. Return to sport: sport-specific drills, cutting and readiness testing.

Each stage takes a different time for each person and follows the surgeon's guidance. Progress is decided by test results, not just by the calendar.

When Is It Safe to Return to Sport?

  • In a cohort study after ACL reconstruction, reinjury risk fell by about 51% for each month that return to sport was delayed, up to 9 months after surgery.

  • Quadriceps strength also matters. Reinjury was less common when the operated leg's quadriceps strength was at least 90% of the healthy leg.

  • That is why physiotherapists assess readiness with strength, hop and sport-specific tests before you return to competition.

Physiotherapy at Good Physio ID

  • Initial assessment and regular strength testing to track progress.

  • Progressive strengthening, balance, and running and jumping programmes.

  • The final stage can continue at our partner gym, MOVA ACTIVE, until you are ready to return to sport.

  • A home programme to keep training consistent between sessions.
