Saraf Kejepit (HNP): Penyebab, Gejala, dan Peran FisioterapiPinched Nerve (Lumbar Disc Herniation): Causes, Symptoms and Physiotherapy

KondisiConditions

  • Sulit menahan buang air besar.

  • Tetap bergerak ringan sesuai toleransi.

A pinched nerve in the lower back, or lumbar disc herniation, happens when the cushion between two vertebrae bulges out and presses on or irritates a nerve root. The good news is that most cases improve without surgery.

What Is a Disc Herniation?

  • Between the vertebrae sit discs made of a soft core (nucleus pulposus) and a strong outer ring (annulus fibrosus).

  • In a herniation, the soft core pushes out through a tear in the outer ring.

  • It most often occurs in the lower back, at L4–L5 and L5–S1.

  • When it presses on or irritates a nerve root, pain can travel down the leg. This is often called sciatica or radiculopathy.

Common Symptoms

  • Low back pain spreading to the buttock, thigh, calf and foot. Leg pain is often worse than back pain.

  • Tingling or numbness in the leg or foot.

  • Pain that increases with prolonged sitting, bending, coughing or sneezing.

  • In some people, leg muscles feel weak.

Warning signs: go to the emergency department

  • Numbness or tingling in the groin, genitals or around the back passage.

  • Difficulty passing urine, not feeling it, or leaking urine.

  • Difficulty controlling your bowels.

  • Pain or tingling in both legs, or legs getting weaker.

  • Pain with fever, unexplained weight loss, a history of cancer, or after a fall or heavy impact.

These may signal cauda equina syndrome or another serious condition. They need emergency hospital care, not physiotherapy.

Do I Need Surgery?

  • Most people do not. A systematic review found that around 63% of herniations treated without surgery shrank (regressed) on follow-up imaging.

  • Larger herniations actually shrink more often: about 96% for sequestration and 70% for extrusion, compared with about 41% for protrusion.

  • Doctors consider surgery when there are warning signs, worsening nerve weakness, or severe pain that does not improve after adequate conservative care.

  • MRI findings should be read together with a physical examination. Treatment is not decided by the scan alone.

The Role of Physiotherapy

  • Assessment: history, muscle strength, reflexes, sensation and nerve tests, to confirm the condition is safe to manage without surgery or needs a doctor's referral.

  • Education: understanding the condition, helpful positions and activities, and staying active within tolerance. Prolonged bed rest is not recommended.

  • Exercise therapy: starting with movements that ease leg pain, progressing to core and leg stability and strengthening, and back to activity and sport.

  • Manual therapy alongside an exercise programme, in line with NICE guidance.

  • Home programme and regular re-assessment to track progress.

What You Can Do Now

  • Keep moving gently within tolerance. Break up long periods of sitting by standing or walking for a moment.

  • Avoid heavy lifting and repeated bending while pain is high.

  • Watch where symptoms travel. If pain spreads further down the leg or weakness appears, get checked promptly.

  • Use pain relief as advised by your doctor or pharmacist.
